It has come to our attention that some of the 2009 1099 Dividend forms were mailed to
incorrect addresses. We regret this error occurred and we are working quickly to correct
the situation. Enclosed is a duplicate copy of your 2009 form 1099- DIV, in the event
you have not received the original. If you received a 1099 that does not belong to you,
please leave it sealed, and write “return to sender” on the outside of the envelope, so that
cost of postage will not be incurred. If you have already opened a form that was
improperly addressed, please destroy it immediately. Thank you for your assistance with
this matter and we sincerely apologize for any inconvenience it may cause you.
